add a function to message me

I want to use COD but in japan we can choose delivery time.

 

I use the free module from presta i simply want to add a drop down box in the template for validation.tpl

 

then when the customer clicks submit it adds a message in the admin page notifying me of the time chosen.

 

anyone able to help a newbie do this?

 

i know how to add a form box but not how to set a script to grab the choice and message it.
