UK shop - few issues any one been there?

I hope one can help me, I am trying to set-up my UK shop, but struggle with a few things.

Yes, I have read the manuals available and googled a lot, but can't get this correct.

 

Shop starts with US as default language, I imported the United Kingdom under localization.

All boxes ticked [states, VAT, currencies and so on] After that I changed the default language to English (United Kingdom) so far so good...

 

But, no States [counties] are imported, what do I need to do to get these?

 

url is showing: www.myshop.com/gb     ??? why not www.myshop.com 

How to fix this or is that not possible because of languages?

 

I made sure date format is in d-m-Y  but on many places it still shows 2013-12-18

 

Any one can help and confirm me with these issues?

For counties, try this thread: http://www.prestashop.com/forums/topic/292159-uk-counties-missing

To drop the 'gb' from the URL, disable (but don't delete) US English so that UK English is your only language.

Not sure offhand about the date format - can you confirm where you set it and where it's not displaying as expected.
