Add text fields in the contact form

Hello guys,

I added text fields in the file contact-form.tpl ... The problem now is what I need to add in contact-form.php? And above all, what? Why is my contact-form.php is as follows:

That tut is very nice, but i need to put an introduction text on contact us page after "Send us a message" text. How can i do this? I have a bilingual site.
