Store locator search field in home page

Hi everyone and thanks in advance for the help you would like to provide!

 

I'm wondering how to add a store search field (just like the one in the "stores" page) to the home page.

 

This search field should load the "stores" page when clicking search button, passing location value to that page so that relative stores list is directly shown.

 

Has somebody already developed a module with similar functionality or is there a paid module somewhere?
