Extra field at registration

Hi,

 

I would need an extra filed at customer registration - where they need to pick a store location (We need this field for accounting purposes). 

 

We have a lot of locations that need to be uploaded with a .csv into this field.

 

My PS version is 1.5.4.1

 

Has somebody an idea?
