Product Page Alignment & 'Notify me when in stock' Error

Please view the attached picture.

 

1. The aligment of the notify me when in stock is off (the email box) what do i have to edit for this?

2. Even when the product is in stock it comes up with this box which isnt required.

I just turned off loyalty points before you must of viewed it. Do you know why it would say notify me when it's in stock?
