prestashop error

I am getting below error on each places rather then homepage. I mean when I am trying to access order page its throwing me this error:

This page is not available

We're sorry, but the Web address you've entered is no longer available.

To find a product, please type its name in the field below.

I tried to regenerate htaccess file but still error there.

Please help.

1) ps version

2) theme that you use

3) you run store with "friendly urls" turned on?

4) have you regenerated .htaccess file?

Ans: 1.5.4.1

2) theme that you use

Ans: Modshop (transformer).

3) you run store with "friendly urls" turned on?

Ans: Yes. i am.

4) have you regenerated .htaccess file?

Ans: Yes. I did it already many times.

 

Also before 1 week site was working fine with no issues. And after I did changes URL rewrites but it was still working. Not sure what happened suddenly.

When I turn off the friendly urls I can see the product, but If I turn On the friendly URL I get an error "
