Create product option (checkbox) not a combination

Hi,

 

I am looking for a solution to allow a customer to "check" a product option on the product page before adding to cart.

 

For example, I want my customer to select if she would like to receive a free gift with a specific product.

 

I was unable to find this option in the backend. I tried creating an attribute for the free gift, but I only saw an option for a radio button. This does not work because it is selected by default, and you cannot uncheck the box.

 

I am looking for any suggestions. Thank you!
