Add a Form before checkout?

Hi,

    I'd linke to know if is possible to add a custom form before checkout?. I need to include a form where the client can design its own label, upload logo...then when client finish payment is processed? Any idea will be appreciated.

i don't know any module to customize uploaded file, but i know modules to upload files (files upload module, customer files upload) etc.

with second one you can turn on uploading file in checkout process (in cart step)
