upgrading from 1.3.2.3 to 1.4.11 throws 2 sql errors

I have just upgraded from 1.3.2.3 to 1.4.11 and got 2 sql erros.  I found one and thanks to this forum have rectified it ( (1091) Can't DROP 'category_product_index'; check that column/key exists) but can't for the life of me even find the other error let alone figure out how to fix it!

 

Can someone have a look to see if they can find the error for me?

I'm not seeing a second error, but I am seeing this...

The () after that line implies an error occurred, but does not say what it was.  The code is removing the blockcategories module from the "afterCreateHtaccess" hook position.  So you could go into the back office and confirm if that was completed successfully, and if not, then delete it manually.
