[Solved] register address firt registration page

Good night!!

I'd like to put the addresses listing on the first screen where the user makes the registry. On the first screen just asks for name, email and password to be registered, and does not ask the address, is to add the address to be registered? Like img file in attach. I tried to edit the form by adding fields but after que page will not load maism then undid the changes.
