Address Registration Problem - Missing Fields

I have a problem with customer registration, when a customer tries to register his/her address they are missing all the boxes for name, surname, first line, postcode, country etc all they see are the fields in the screen shot.

I have not got a clue what is wrong, i checked my error log and have a few htaccess files, i also had a bug report in the file that Valérie Assetskaya mentioned. so still not got a clue whats wrong.

Hi all some advice, i have solved my problem, it was to do with Localization > States there was 2 other states and i removed the person who registered when you start presta up, i made sure me en = English was all same and me GB = Great Britain was same, that was what my problem was,

All fixed, imported new localization pack and done as above
