Show new custom page header & footer

I have created a new module with a custom page but I cannot get the header & footer to show.

I followed the instructions in the 1.5 developer guide but they include

include('../header.php');
include('../footer.php');

which is depreciated in 1.5

All the other posts I found appear to be incomplete or relate to earlier versions.

 

Any help would be appreciated.
