Implementing a drop down menu next to carrier?

Hello, so I would need to implement a drop down menu next to a carrier so the costumer can choose from a list wich postoffice they would like the pack to be delivered to?

 

There is a english tutorial in the end of this page http://smartpost.ee/widget/

I would need to implement the "Minimal version".

 

But don't know exactly how to do it or where to put the code, so can anyone help me out?
