Address formatting for the US and Missing States

We are switching from OSC Max to PrestaShop. We have imported customers and orders. Most orders are US but we do have international orders too.

 

We're having problems with the correct address formatting, and the states are missing from both the orders and customers addresses.

 

We do have localization set for the US, as the default. However, under countries > edit... we can't get the formatting to update... it simply won't hold the updated formatting.

 

From this

Any ideas on how we might fix this?
