How to add custom fields to checkout

I'm running Prestashop 1.4.7.0 and I need to add 3 custom information fields into both 5 step checkout and guest checkout. I only found info about how to remove fields from checkout, but almost no info about how to add custom information fields. Informations collected from these fields should be visible somewhere in BO in information about customer.

 

I need something like this:

 

Name:

etc.

etc.

Information 1:

Information 2:

Information 3:

 

Any suggestions?

Hi Alexander, can your module also change guest checkout? I was playing with the demo but I didn't find such option. Thanks.
