Need custom adaptation of One page checkout look

I'm using the one page ceckout built in the PS 1.4.5.1 verision and would like to have it look different than it does now.

 

Have a look at the attached screenshots of the current look and how I want it to look. Please note the after screenshot is just a very crude version, not the final one. I wish to keep my current website theme, just to change this one page.

 

I would like this change/customization be compatible with 1.4.5.1 And 1.5

How has this worked out.

 

I would like to know as the custom one page check out makes to many problems and make to many customer to leave.
