Looking for customer registration customization module

Does anybody know of a module for version 1.5 and higher that will allow the customization of the customer registration page?

 

I would like to be able to edit and add new fields from my back office,

for example: Sales & Use tax id, EIN, Banking info, Subscribe to Newsletter, Person in charge, Best time to call, etc.
