Problem with new order invoice generation and cart synchronization

We have been using Prestashop 1.4.7 for the last 5 months to this day without any issue. However, since last few weeks, we have been having problems with invoice generation as new order is passed. The problem is seen randomly among customers choosing to pay via Paypal. The problem goes like this:

 

- In the Dashboard of back office, in the block where list of recent orders are displayed, i can see that a client has made an order but in the status, i do not see any order status.

- I confirmed through Paypal that this particular client has paid the total amount and also the list of products he has ordered (in paypal).

- In the order tab, this particular command is not displayed, however when i modifiy the id_order in the url of the order detail page, i can see this particular order id page detail.. here i find that that no invoice has been created for this order and also the list of product ordered is null. However, i can see that the payment has been through paypal and somehow there is total for the order displayed along with cart id.

- If i follow this cart id and look up the cart list in the client tab, i can see the cart created by this particular client and the list of product ordered matches exactly to that of a copy with paypal.

- Also i didnt get any email about new order for this order.

 

So, somehow when a client went to paypal to pay for the order and when he came back to site, this problem might have occured. First I thought it might be problem with server outage, as we were occasionally having problem with server downtimes.. so i have migrated the whole site to a new reliable server and its comparatively very fast and much reliable. But, the problem persists and it is very occasional. It only happens in some order, it has been happening few times a week.

 

I would like to know if there is anyone having similar problems with prestashop and or any ways i can debug this problem. I have uploaded some snapshots..
