"Request a Quote" - which modules?

Hi guys,

 

 

I am searching for a "Request a quote" module for a catalogue-style shop.

 

Wanted features:

- request form

- database storage of request

- email of request to staff

- autoresponder to customer

 

That's a lot of stuff in there... :-)

 

Do you guys know of any modules which are compatible with multishop 1.5.2?
