Change module for shipping and carrier

Hey everyone,

I desperately need help on this. I want something like this.

1)When user addes any product to the cart.

2) Cart will first ask to sign in

3) After sign in a customer fills the form

4) Then after that a message will appear as we will call you to confirm this delivary.

 

I don't want any shipping or carriers, I just need to display this message. So guys anyone please let me know how to do that.
