custom form under product link

Hi,

 

I would like to have custom form

 

i.e. when i click on say particular product category i would like the form to appear with custom form in the custom page with the information i would like to display on the page together with the form.

 

On the same form i should be able to make mandatory fields and on submission i would like to display a custom message and generate two emails

 

one email goes to administrator / specified email address

 

and another email goes to the user/query initiator.

 

any help will be appreciated.
