Adding a custom form

We want to add an appointment booking form with a custom calender and fields to our Prestashop.

 

Adding text pages is fine, but how to add a custom form that stays with the store? We added an external page, but the shoppers can't stay logged in if they leave the storea area.

 

Any help or pre-existing modules would be much appreciated.

Form part is not a big deal, but how complicated is your custom calender ?

 

What are the choices ? Day + Hour + minutes...

Who and how will you validate appointment and then remove the time zone concerned ?
