Problem in adding the product to cart from product page

Hello,

 

I am developing a new website for one of the contact lenses company. And i am facing some problem. Any help regarding this is much appreciated.

 

1) I am able to add the product for one eye but the problem is with the help of checkbox the another eye should also be added from the same product page.

 

Example: There is a XYZ product. And it has ABC attributes. From the product page i am able to select the particular attribute and add to cart the product for left eye but not been able to add the product with different attribute for right eye.
