How to expand order information

Hi all,

 

I am still new and I found that I've just got a more challenging problem, so need your help

 

The thing is that I have added a new checkbox which is showed when client is making order (exactly in step 3 called "address"). I have also add new column to database (where orders are stored).

 

And the last thing is to make it flow through page

 

ANd here is my question, where should I change order to store one more variable?
