Auto add additional line item

I am trying to find a module or suggestion for the following problem:

 

I setup an item for sale for an auto parts store. I setup an attribute list called "core return" with options of "please select", "yes" and "no" to select from the drop down. The store does not want to use the ability to track inventory as 99% of items are drop shipped to customer. I can setup the attribute relations just fine to have it increase the cost upon selection of the attribute.

 

Problem is if the customer does not make a selection, it leaves the default of "please select" and continues with the order. What I need is a solution, however it may come. Ideally, the customer cannot "add to cart" until selection has been made. Also, it would great if the extra chage (in this case a "core charge" for an automotive part) could automatically be added as a second line item on the checkout screen so the customer knows the amount of the charge in plain text. It will also make it easier for them to then know what refund amount to expect upon return of the core product.

 

I have also tried to make a product called "core charge" and add it to the item as a pack in the product setup. The problem I find here is that then both products are available, the one with the core charge and the one without...

 

I am not sure if there is a workaround, but I would love to know if someone else out there has come up with a solution to this problem. I am positive with the pool of talent out here, someone can share some insight.
