[Help] Modul testimonial error...

Saya menggunakan prestashop 1.4.8.2

modul testimonial saya sdh download dan install modul testimonial :

tetapi pada tampilan web (local host) ketika saya klik 'Add Your Testimonial' maupun 'View All Testimonials' selalu error..

saya sudah mencoba blocktestimonial versi 1.4, versi 1.4.1, versi 1.4.2

semuanya tetap error..

 

error muncul pada saat saya klik 'add your testimonial' :

mungkin bisa di cek di error_log apachenya.

 

tapi ati2 aja kalo dipake live, baeknya dipasangin captcha juga, yang udah2 dibom spam via halaman add testimonial.

saya cek pada :

C:\Documents and Settings\Administrator\Start Menu\xampp\xampp\apache\logs\error.txt

 

error logs nya seperti ini:

saya sudah setting favicon.ico nya

tapi ketika saya klik halaman testimonial baik add new maupun view all testimonial, hasilnya pada

http://localhost/prestashop/modules/blocktestimonial/addtestimonial.php

 

yang muncul halaman kosong (blank page)....

pesan erornya ga membantu.. btw, kalo di config/config.inc.php si display_error jadi on ada pesan yang lebih jelas ga selain page blank ?

sudah saya rubah on display_error nya, hasilnya tetep blank page...

di log gak ada error
