Still Having Image Problems

Can someone just PLEASE give me instructions on how to upload images via .csv file? I have done everything I have found in these forums to do. I have uploaded the images to a folder on the server, redirected the URL's in my .csv file to reflect that...and NOTHING HAPPENED. I get an error that says it had trouble copying the URL--even though when I click on the hyperlink in my .csv file it shows up just fine--and I get nothing to show up on my site. when I go to a product in the catalog, it says the product has an image, but all I get is a box with a gray question mark in it. This has been going on for months now and I can't find anything online or anyone to help.
