I Am Looking For A Form

Hello,

I already have a website running on Prestashop 1.3.6.0. I sell guides related to different universities. I am now planning to take custom orders as well. For this I need a form that takes email, information regarding the work, number of pages, different deadlines and rates calculated according to the number of pages and the deadline. That calculated payment then be proceeded to checkout using the default settings of the payment of the website. I need this in a module. If anyone is interested then please let me know as soon as possible.
