How to recive Some payment details from customer,like check nuber or upload check image to confirm

I need to get some information about while check payment like check no ,or an image of check to be uploaded by the order to reduce order process

 

is there any way to do this

Hey, if you don't mind me asking: What module did you create? I was looking through the Cheque module code and was somewhat overwhelmed. I know PHP, but I can not make sense of it. I am trying to make a module that just takes the user's bankcard info and stores it for later review and processing (offline).
