Self coded page with current prestashop theme

Hi all.

I wanna create a self coded page with php that renders a form but my problem is that i prefer that the theme of that manual page be current prestashop theme.

for example I don't know whether you have seen about us page or not but about us page indicates some manual notes but with theme of the current prestashop theme.

I wanna create a webpage by php and render a form in it but I force it to use prestashop theme.

what to do?
