Problem with my contact form

Hi - anyone who can help a NOVICE in PS?

 

When using the contact form in my shop - i get a failure of http 500

 

I did this:

Through Employees - Contact, I made this new contacts: Webmaster and Customer Service

 

Through Preferences - E-mail, I made these settings: php and both

 

 

I can actually send mail through the e-mail tester - but once on my website and trying to mail the contact form i get the failure.

In SEO&URL have you checked friendly URL and other URL rewriting option ?
