Using own contact form php script

Hi Guys

 

I want to embed my own custom php contact form script into my website. I tried adding the code from the form to my template file & even tried including it within the cms page but all failed.

 

My client is very particular about the contact form & how he receives emails so I need to use this and not the default form.

 

Any idea how I can accomplish this?
