Cart - No product image and out of stock red dot!

I am really stuck with a cart issue and would appreciate some help and guidance.

If I add a product in the cart and then click on the cart button (in the right hand column) to view my cart contents, all I see is a missing product image and red icon (which indicates out of stock)

I know the product is in stock and there is a valid image. I have tried clearing the cache and cookies etc but to no avail.
Everything else seems to be working fine – please help.
