Creating a CMS Page for Catagories Module

Listed on the left side of my webpage is the module that is titled "Catagories". Within there I have listed sub-catagories of products. What I want to do is create another sub catagory called something like "Build one for Me". When clicking on that link, I want to have it open a page ( I think that is a CMS page) which will contain a form to fill out.

 

I have the form for the page but can't figure out how to attach it to "Build one for Me" link under "Catagories". I can do it easily in the "Information" CMS module, but I don't want it there.

 

If you can, please give specific instructions as I am still learning here.
