I Have two problem with logout and validation. please help me

Hello friends

 

I Use prestashop 1,3,1 . when my shop users want to logout from site , show white page and this message :

 

Warning: Cannot modify header information - headers already sent by (output started at /home/pmarketi/public_html/header.php:2) in /home/pmarketi/public_html/header.php on line 5

 

Warning: Cannot modify header information - headers already sent by (output started at /home/pmarketi/public_html/header.php:2) in /home/pmarketi/public_html/classes/Cookie.php on line 261

 

and other problem ,

dont show the plan account information for customer (validation.php) and show white page with this error :
