I would like to include a service at the checkout point. Similar to the Gift Option.

I sell amber jewellery for healing and the extra I want to include at checkout is a checkbox should a customer wish to include a service of Cleansing, Charging and Programming the crystal jewellery before shipping. This being an extra fee of £3.00. If ticked then a box to appear where they can write a note (similar to Gift Option) and then the cart to recalculate to include this service fee.

Any ideas please on how I go about doing this.
