At the closing of the purchase, email data.

Hey everybody, okay?

So I need to do something like this:

When you close the client application and make payment for, the system opens a form to register their customer data.
Once you complete this form, the system will send an email with all that data to my email.

My client will not use any payment module in his shop, he will receive an email with all the data address / card customer in the store and there it has the credit card machine .. it will put the numbers of cards in the machine and will make the purchase by the host.

How can I do?

What I think is just create a popup in html / php even opening a form and send via mail (). No secret ..

But how has it right in the same store?
