Parece que el usuario no tiene permisos para interactuar con la base de datos. Antes que nada, asegúrate de que el usuario en cuestión es legítimo, luego ve a PhpMyadmin (o lo que uses para gestionar tu BD) y dale permisos, puedes hacerlo manualmente o ejecutando un comande como este:

GRANT INSERT ON `nombre_de_tu_base_de_datos`.`r8wf_guest` TO 'o4474508'@'infong1022.perfora.net';
FLUSH PRIVILEGES;

Asegúrate de cambiar el nombre de la base de datos por el que tú tengas!
