Custom Form Adding and Connect with Payment

I want to add fully Customize form, then that customize form data should be save on database. when submitting custom form data it should redirect to order page and after payment done it will save. 
now I have done these task

• custom form design
• add designed form to home page
• save form data to database.

now I have to do

1. when save that data it should be redirect order page and payment done it will save or saved form table field status change.

these are the requirement, how to do that?
