Querying Stores

Anybody know a quick and easy set of parameters to query stores.php. I'm trying to create a simple box for "Find a Retailer" that will take in a zipcode and pass it to the page where a result can be found. Before all the ajax and javascript we used to call this a <form> ;-)

I've tried all the basics but something isn't meshing and instead of spending a lot more time on something somebody else has probably already done I figured I would ask.
