Looking for a Form Builder with Conditional Fields?

Hi there,
I am interested in finding a good form builder module [free or paid] which allows me to show hide some fields based upon certain conditions? Is there any such thing available for Prestashop at all? such form builders are available for WordPress but I couldn't find one for Prestashop.

I will really appreciate any pointer in this regard.

@SmartDataSoftI have had a good look at both of these products BUT unfortunately, none of them provides the ability [I am looking for in the form builder] to show/hide certain fields on the form based upon an other field value? like if that particular field is empty, or contains a specific value.

Did you find any form builders or did you use something more general? 

I could not find any module with conditional logic forms. 

 

Let's say if a user selects B from dropdown list of A, B, C then a new set of selections and text will appear under it compared to if they had selected A. 

 

There are such apps for Shopify so it is a shame none exist for Prestashop.
