CSASupport
You say you do not see the TOOLS folder, correct?
theme_designer
You can just set all folders permission to 777.

tdr170
What are you uploading to php myadmin, I also use Ipage the only thing you need to do there is create a database. Go to the control panel click on MySQL Database and create a database with a name of your choice. Also Ipage does not seem to like the localhost as the database server so use the actual server name - yourdomain.ipagemysql.com When you upload the files with filezilla are you uploading the files inside the unzipped folder or the folder itself. If you want prestashop to be the main domain just upload the files inside the unzipped folder, if you want prestashop to be a sub domain you can create a folder named shop and upload the files to that folder or just upload the prestashop folder. If you upload just the files the url will be www.yourdomain.com/install, if you create a sub domain folder named shop the the url will be www.yourdomain.com/shop/install if you upload the prestashop folder the url will be www.yourdomain.com/prestashop/install. I hope this gives you a better idea of the steps needed to install Prestashop.
