Covid 19 declaration form

I run a club website and are looking for a way for members to 'declare' they are free of Covid symptoms before attending an event.  

So it would be:

- Name

- Answer four questions

- Press 'Submit' which sends it to a specified email address

Has anyone found a module to do this - I have looked at the Webkul module which looks ok, but any opinions appreciated.
