E-shop at the end of the order "cash on delivery" drops

Hallo,

 

could you help me please somebody with problem of ending order on my e-shop www.light-market.cz. When I confirm order by "cash on delivery" - it drops (fall down) and writes:

but what it exactly means. I can open validation.php over FTP, but I do not know what exactly change there.

Only text "template" including is        $this->setTemplate('validation.tpl');
