Extra product options with price influence

I am looking for a module where I can add checkboxes, dropdowns or text fields to customize a product.

Example: I have a wooden box and I want customers to have the option to check an extra option "Label on product". This label costs €2,50 and it should be available for different types of products.

Next to this I want the customer to be able to specify the label by choosing a font and the actual text on the label.

Note: This does not have to be visualized on the product images.

 

I bought a module already "Additional Product Attributes / Custom Product Fields", but this module does not seem to function well in my webshop, so I am looking for an alternative with similar functionality. Any suggestions?
