"cash to delivery" with specific characteristics

Hi,

we are looking for a module with this feature:
decise a specific client to add this option (for trusted customers who buy often)
decise for which courrier add this option (not every courriers accept "cash delivery")
Add a cost (2% or more it depend of the courrier).

Any idea?
