Images twice upload make double product in cart

Hello to everybody, I have a problem with images and products.

When I upload images it will be duplicate, in database ps_image, ps_image_shop there is just one row.

If I delete one image, will be remove both, this because is just one.

This is make big issue in frontend, because when some customer add one product will see two same products in cart (two rows like added two different product) 

Prestashop version is 1.6.1.24 and is an upgrade from one of the firsts Prestashop 1.6 version.

It is make me crazy, how can I solve it?
