Product combination doesn't work with the Blockcart (v 1.3.6) on Chrome, IE & Safari

Hi prestashoppers,

This is my first time working a project with prestashop. I customize the template and created some modules too. However I have problem with product combination that only works on Firefox and Opera browsers, but not in Chrome, IE and Safari.

Try to add the product to cart using different combination (ukuran = size, it's an Indonesian site). It always add the default combination, not the desired combination we choose from the selection.

I think it's something with the javascript, but I still can't figure it out. The js error console showed no error messages.

I use Prestashop 1.3.6. Thanks for helping me out or pointing me to the right direction. Thanks.

I think it's something with my new customized theme because I have tested using the default theme and it worked seamlessly, but still I hope somebody can give me a clue about this. Thanks.

Here's the screenshot. I have compared the html source with the original Prestashop template but still can figure out. Please help. Thanks.
